WELCOME NEW PATIENTS! Completing patient forms in advance speeds the registration and check-in process, so please download and complete the forms applicable to your visit prior to your appointment. If you are unable to complete the forms in advance, please arrive 30 minutes prior to your appointment to complete the forms in our office. It’s also critical that you review the checklist below for a list of items to bring with you to your appointment.

Click on the Medical Record Release Authorization link to download the release form. Complete the form and provide it to the medical office you’d like to request records from, and follow-up with this office to ensure that your records have been mailed or faxed to Prestige Pain.
